Good Faith Estimate Notice

Under federal law, healthcare providers are required to provide clients who do not have insurance or who choose not to use their insurance with a Good Faith Estimate of expected charges for services. You have the right to receive a Good Faith Estimate explaining the anticipated cost of services before receiving care. The estimate is based on information available at the time it is prepared and is intended to help you understand your expected financial responsibility. Please note that a Good Faith Estimate is not a contract and does not obligate you to receive services. Actual costs may vary depending on your individual treatment needs and the number of sessions required. If you receive a bill that is substantially higher than your Good Faith Estimate, you may have the right to dispute the charges. You may request a Good Faith Estimate at any time before scheduling services or during the course of treatment. For additional information about your rights under the No Surprises Act, please visit www.cms.gov/nosurprises.
